This "fit-in-your-pocket" product from Weatherbee Resources includes all three parts of the Local Coverage Determinations (LCDs) for the following diseases: ALS, Dementia/Alzheimers, Cancer, Heart Disease, HIV, Liver Disease, Pulmonary Disease, Renal Disease, and Stroke and Coma; and Neurological Conditions and Cardiopulmonary Conditions.

 

Four assessment scales are included: the Functional Assessment Stage (FAST), Karnofsky Performance Scale (KPS), Palliative Performance Scale (PPS), and the New York Heart Association Functional Classification (NYHA). The Palmetto version includes the International Classification of Functioning, Disability and Health (ICF) scale.
